Job description

On-Site Clinical Nurse (RN) – Home Care

Position Summary

The Clinical Nurse Manager (RN) is a salaried leadership role responsible for overseeing clinical quality, caregiver supervision, and client care management for SmithLife Homecare at a primary assigned location. This position provides clinical oversight, assessments, and training while supporting continuity of care and regulatory compliance.

Travel is limited and primarily occurs only to provide coverage during scheduled PTO or absence of another clinical team member. Travel will be around ‘campus’ only.

Employment Type: Salaried, Exempt
Primary Location: Single assigned office location
Reports to: Administrator
Schedule: Monday–Friday, Day Shift

Qualifications & Requirements

  • Associate Degree in Nursing required; BSN preferred
  • Active, unrestricted Registered Nurse (RN) license in the State of Maryland
  • Experience in home care, home health, long-term care, or related healthcare setting
  • Strong clinical judgment, assessment, and documentation skills
  • Organized, detail-oriented, and able to manage responsibilities independently
  • Professional, collaborative, and solutions-focused communication style

Key Responsibilities

Clinical Oversight & Assessments

  • Oversee delivery of skilled, personal, and companion care services at the assigned location
  • Conduct initial nursing assessments for new clients and develop Care Plans within 24 hours of admission
  • Perform routine supervisory visits and reassessments every 90 days
  • Complete supervisory visits every 45 days for clients with complex or skilled needs, including high-risk equipment, medication administration, or multiple co-morbidities
  • Perform assessments within 24 hours for clients returning home from hospitalization when services resume
  • Complete additional assessments and Care Plan updates for any change in condition or as clinically indicated

Supervision, Training & Quality Assurance

  • Observe, evaluate, and document caregiver competencies during home visits
  • Provide training, retraining, and skill reassessment for CNAs and HHAs
  • Review client charts, caregiver documentation, and progress notes; implement corrective action plans as needed
  • Ensure all clinical documentation meets regulatory and agency standards

Care Coordination

  • Collaborate closely with the Administrator on client admissions, service planning, and care quality
  • Communicate with physicians and interdisciplinary care team members as needed
  • Provide clinical support for client needs requiring RN involvement, including medication support and urgent care coordination

Compliance & Incident Management

  • Participate in investigation and documentation of incidents, accidents, or changes in client status
  • Support abuse and neglect investigations in collaboration with leadership and report to appropriate authorities as required
  • Prepare and distribute discharge notices in compliance with COMAR regulations
  • Assist with infection control response and public health events as needed
