The program brings together a team of professionals, family members, and community resources to develop and implement a personalized plan of care. The goal is to ensure that all aspects of a person's life—emotional, social, educational, and physical—are addressed in a coordinated and holistic manner. Wraparound Programs are typically used for those involved in multiple service systems, such as mental health, child welfare, and juvenile justice.
