Description:

This position is a HYBRID position supporting the primary care in Spencer, IN and virtual support for other offices within the region.  The position will work part-time in person at the Spencer, IN office and part-time virtually.  Candidate may select from working 5, 8-hour days or 4, 10-hour days.

Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, medication therapy monitoring, regulatory compliance, and protecting the confidentiality of patient information. Provides medication information support for medical and nursing staff, ensuring continuity of care, and identifies potential challenges and intervenes, if necessary, to medication-related issues.

  • Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
  • Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
  • Requires 0-3 years of relevant experience.
  • Requires current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
  • Board certification or other specialized training may be required.
  • Basic Life Support certification through the American Heart Association (AHA) may be required.
