Fathers amp; Families Support Center
Position Title: HomeGrownSTL Life Coach Reports To: Quality Assurance Manager
Job Status: Full-Time / Non-Exempt Direct Reports: N/A
Description of this role: Responsible for providing dedicated and extensive life coaching, case management and community resource navigation to HomeGrown StL project participants. Life Coach will provide one-on-one services towards improving the economic and upward mobility outcomes of fathers ages 18-35 in the greater St. Louis region. Project ends March 31, 2027.
Duties, Functions and Responsibilities - includes the following and other related duties may be assigned.
  • Provide dedicated and extensive life coaching, case management and community resource navigation to caseload of Project participants through in-person and virtual meetings for at least one year.
  • Completes a social assessment, which includes personal interviews as well as collection of secondary information to complete an individualized plan for self-sufficiency and responsible fatherhood.
  • Research, contact and build relationships with community resource agencies, businesses and institutions to connect and refer Project participants to meet their needs.
  • Work directly with HomeGrown StL partners and FFSC Quality Assurance Manager to co-create and implement Project’s Life Coaching cohorts and Community Resource Partner presentations.
  • Facilitate in-person Life Coaching cohorts featuring Community Resource Partner presentations over the course of eight weeks and two-hour Sessions.
  • Prepare and present electronic progress reports and caseload summaries during biweekly Status meetings.
  • Assist with Project outreach, recruitment and enrollment through in-person and virtual presentations.
  • Attend mandatory all staff meetings, Project training sessions, and community and networking events.
Knowledge, Skills and Abilities:
  • Must be able to manage high-volume caseload of up to 20 participants.
  • Must have effective verbal and written communications and interpersonal skills with ability to multi-task.
  • Must be able to utilize agency and community resources to meet goals. Familiar with MS Office Products.
  • Must be able to organize and present information as well as work independently.
  • Must have strong case management and documentation skills.
  • Must understand and practice the NASW code of ethics.
Educational and Experience Requirements:
  • AAS or BS degree in Human Services or related field or HS Diploma/GED plus 5 years of experience working with at risk populations. BSW, MSW preferred.
  • Must have experience in working with at-risk populations and prior work with group facilitation.
  • Must be able to work flexible schedule with some evenings.
  • Must possess a valid driver’s license and have a reliable, insured vehicle.
