Job description

Job Title: RN Desk Case Manager
Department: Homecare
Location: Rochester Regional Health Home Care - Monroe Ave
Schedule: Monday-Friday, 8am-4:30pm
SUMMARY:
The Desk Case Manager manages the delivery of care to a set of assigned patients during their home care admission. This role involves coordination with the care team and combined with virtual contacts to assess the needs of patients and their families. Responsibilities include developing, implementing, evaluating, and adjusting an appropriate Plan of Care based on the patient's clinical needs. The Desk Case Manager identifies appropriate interdisciplinary services needed, and coordinates required services.
RESPONSIBILITIES:
  • Identifies and prioritizes health problems based on in home assessments with additional virtual contacts, as well as report from the care team and clinicians performing in-home visits.
  • Develops and facilitates an patient centered interdisciplinary Plan of Care based on the needs identified during the assessment taking into consideration input from the patient (and their caregivers as applicable) as well as clinicians performing in-home visits
  • Plans, organizes and prioritizes care needs for an assigned caseload of patients to ensure their care needs are met and services are delivered according to plan of care
  • Schedules nursing visits at appropriate frequency based on clinical need
  • Aware of and considering the timing of episodes and periods for effective LUPA prevention
  • Recognizing and managing the risk of hospitalization
  • Keeps track of medical appointments and follow-up requirements.
  • Notifies the relevant care team members of any changes in patient status or service needs and ensures that timely and appropriate actions are taken.
  • Facilitates the development and implementation of patient discharge plans
  • Documents all patient care and coordinating activities per agency standards
  • Practices according to Agency and community standards
  • Consistently demonstrates high standards of integrity by supporting the Rochester Regional Health Companies' mission and values and adhering to the Corporate Code of Conduct.
  • Maintains high regard for patient privacy in accordance with the corporate privacy policies and procedures.
  • Regular and reliable attendance is expected and required.
  • Performs other job-related duties as assigned by management.
REQUIRED QUALIFICATIONS:
  • Diploma or Associate's Degree in Nursing required
  • A minimum of 1 year experience of home health, clinical or direct patient care
  • Current Registered Nurse license in New York State.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
