Job description

The Case Manager provides coordinated, trauma-informed case management for youth served through Project RISE: Youth Victim Support & Healing Initiative. Reporting to the Program Director, the Case Manager serves as a primary point of contact for youth and families, completing intake and needs assessments, developing individualized service plans, coordinating referrals, supporting family communication, and providing consistent follow-up. The Case Manager works closely with the Victim Advocate, schools, mental health professionals, and community partners to reduce barriers to services and strengthen continuity of care for youth impacted by violence and trauma.

Essential Duties and Responsibilites
  • Complete participant intake and trauma-informed needs assessments to identify victimization-related needs, strengths, immediate concerns, service barriers, and appropriate supports.
  • Develop and maintain individualized service plans based on participant and family needs, goals, referrals, and identified priorities.
  • Maintain regular contact with youth and caregivers to monitor progress, changing needs, and follow-through on service connections.
  • Coordinate with the Victim Advocate and Program Director when safety, crisis, or higher-level support needs are identified.
  • Provide ongoing case management, resource navigation, referrals, and follow-up for mental health, school-based, victim-service, and community supports.
  • Work with families to reduce barriers to care, including challenges related to transportation, service navigation, communication, and access.
  • Coordinate with schools, mental health providers, and community partners to improve continuity of care while maintaining appropriate confidentiality and consent.
  • Support warm handoffs and follow-up so participants are not left to navigate disconnected systems on their own.
  • Maintain timely, accurate, and confidential case notes, referral records, service plans, contact logs, and other required program documentation.
  • Track participant engagement, referrals, service utilization, and follow-up outcomes in accordance with ROC ROYAL and grant requirements.
  • Communicate participant needs, service barriers, and significant concerns to the Program Director and appropriate team members.
  • Participate in case consultations, staff meetings, partner meetings, trainings, and program evaluation activities as required.
Qualifications
  • Bachelorโ€™s degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, Counseling, or a related field preferred; an Associateโ€™s degree with relevant case-management experience will also be considered.
  • Minimum of 2 years of experience in case management, youth services, victim services, behavioral health, family support, or community-based human services preferred.
  • Experience working with youth and families impacted by trauma, violence, instability, or barriers to accessing services strongly preferred.
  • Knowledge of community resources, referral coordination, trauma-informed care, confidentiality, and culturally responsive practice.
  • Strong interpersonal, organizational, documentation, communication, and follow-up skills.
  • Ability to build trust with youth and caregivers while maintaining professional boundaries and consistent case-management practices.
Additional Requirements
  • Must pass required background checks and meet all ROC ROYAL requirements for working with youth.
  • Ability to travel locally between ROC ROYAL, school, partner, and community-based service locations as needed.
  • Must maintain participant confidentiality and complete documentation accurately and on time.
