The Case Manager in an Assisted Living community is responsible for coordinating resident care and ensuring residents receive appropriate services that promote safety, independence, and quality of life. This role serves as a liaison between residents, families, healthcare providers, and community staff to ensure care needs are identified, communicated, and met. The Case Manager monitors resident status, assists with admissions and transitions of care, maintains accurate documentation, and ensures compliance with applicable regulations and community policies while advocating for resident well-being. The Case Manager must have knowledge of Medicaid programs and Social Security benefits to assist residents and families with available resources and eligibility guidance.

Requirements:
