This travel role focuses on case management, involving certified home health care experience and a comprehensive understanding of EPIC or Netsmart systems. The work is primarily performed during day shifts in a supportive, patient-centered environment.

Key Responsibilities of the Registered Nurse Case Manager
  • Develop and revise plans of treatment and discharge plans in collaboration with patients, families, and care teams.
  • Complete and submit OASIS assessments, reassessments, and other necessary paperwork within defined timeframes.
  • Process orders and notify physicians of patient needs and changes in condition.
  • Provide an environment respecting patient privacy, confidentiality, and property.
  • Initiate preventive and rehabilitative strategies and provide specialized nursing services.
  • Consult with and educate patients, families, and team members on disease processes and self-care strategies.
  • Supervise LPNs to ensure patient health, safety, and adherence to the care plan.
  • Ensure completion of clinical records as required by state and federal legislation.
Requirements of the Registered Nurse Case Manager
  • Certified Home Health Care Experience Required
  • EPIC or Netsmart Required
  • OASIS Experience Required
  • Telemetry Experience Required
  • BLS Certification
  • RN License (NY License ONLY)
  • One year of hospital clinical nursing experience and 9 months of home care experience preferred
  • Current NYS driver's license
  • Critical thinking, clinical assessment, and strong communication skills
  • Ability to work independently and adapt to change
