Job description

Endeavor Health Services is currently seeking a compassionate and dedicated, full-time Targeted Case Manager/Psychosocial Rehabilitation (TCM/PSR) Specialist to join our dynamic, growing team. In this vital role, you will have the opportunity to make a significant impact on the lives of individuals with mental health challenges by providing comprehensive case management and psychosocial support. Your expertise will help our clients navigate their paths to recovery, fostering hope and promoting independence through personalized care plans and therapeutic interventions. This position will be based out of our clinic located at 150 Mt Hope Ave, Rochester.  This location serves forensically connected clients from a variety of referral sources including US Probation, the Bureau of Prisons, NYS Parole, County Probations, the Courts, etc. Candidates must have an interest in working with a diverse population of adults with mental health and/or substance abuse issues including the forensics population.

Responsibilities

The TCM/PSR Specialist:

  • Provides support for the wellness and recovery of clients with complex or chronicle behavioral health issues through targeted interventions
  • Provides referrals and linkage to community services 
  • Assist clients in overcoming behavioral health issues interfering in the client’s ability to function independently in the community
  • Provides advocacy for family support and required services, through management and monitoring processes
  • Requires the ability to work with child, adolescent, and adult clients

Qualifications

  • Bachelor’s degree in a Human Services field required, Master’s preferred
  • Displayed experience in a similar position
  • Electronic health records software experience (Cerner) preferred
  • Excellent communication skills required
