Now Hiring: Travel RN Case Manager – Home Health

Take your nursing career into a rewarding home health case management role with this 13-week travel assignment. This opportunity is ideal for an experienced RN with strong Certified Home Health, OASIS, and Telemetry experience who can work independently and coordinate comprehensive patient care.


Position Details

  • Position: Registered Nurse (RN) Case Manager

  • Specialty: Case Management / Certified Home Health

  • Assignment Length: 13 weeks

  • Schedule: Monday–Friday

  • Shift: 8-hour day shifts

  • Hours: 8:00 AM – 4:30 PM

  • Weekends: As needed

Minimum Requirements
  • Active NY RN License

  • Minimum 1 year of clinical nursing experience in a hospital

  • Certified Home Health Care experience required

  • OASIS experience required

  • EPIC or Netsmart Required

  • Telemetry experience required

  • BLS certification – any provider accepted

  • Current New York State driver's license

  • EPIC or Netsmart experience preferred/highly desired
