Job description

The Position:

St. Patrick Center, a ministry of Catholic Charities of the Archdiocese of St. Louis, is looking for an Intake and Retention Specialist to coordinate all HERO Program intakes and admissions, collect required documentation, and provide followโ€‘up case management for up to one year for clients transitioning to permanent housing. Using a wrapโ€‘around, clientโ€‘centered approach, the Specialist supports housing stability, selfโ€‘sufficiency, and community integration by coordinating individualized services, collaborating with clients, families, natural supports, and providers, and tracking progress toward goals outlined in the Individual Service Strategy/Treatment Plan of Care (ISS). The core working hours for this position are Monday through Friday 8am to 4:30pm.
