Regular communication with team members through video conferencing and digital platforms will be essential in coordinating care and enhancing service delivery.

• Conduct in-depth assessments to evaluate clients' social, emotional, and environmental challenges, employing evidence-based practices to inform your interventions.
• Generate tailored service plans that outline specific goals, intervention strategies, and methods for progress monitoring, ensuring alignment with each client’s needs.
• Provide individual and family counseling, utilizing therapeutic techniques to guide clients through mental health and situational challenges.
• Facilitate crisis intervention strategies, including risk assessment and safety planning for high-risk individuals, providing immediate support and stabilization.
• Maintain collaborative relationships with community agencies and organizations, advocating for clients’ needs and coordinating necessary services.
• Engage in active community outreach initiatives to increase awareness of available resources and agency services, fostering stronger community ties.
• Participate in case conferences, team meetings, and training sessions to share insights and enhance team effectiveness.

Our client population consists of individuals and families facing significant social challenges, such as those impacted by chronic illness, mental health issues, and socio-economic hardships. As an LCSW, you’ll focus on providing empathetic support and professional guidance to help them navigate these obstacles, ensuring they have access to necessary resources and mental health services.

• Master's degree in Social Work from an accredited institution.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated understanding of social and economic justice issues, particularly within underserved communities.
• Strong foundation in trauma-informed care, with the ability to apply this knowledge in practice.
• Exceptional communication skills, with a proven ability to engage diverse populations through cultural competence and active listening.

• Previous experience within a social service agency, mental health clinic, or similar environment, especially with crisis intervention.
• Familiarity with community resources and support systems relevant to social welfare.
• Experience in developing and implementing service plans that respond to individual and family needs.
