This role is fully remote, allowing for flexibility in your workday while maintaining regular business hours. However, some evening and weekend availability may be required based on client needs and program activities. • Conduct in-depth assessments to thoroughly identify clients' social, emotional, and environmental challenges, ensuring that each client's unique circumstances are understood.
• Develop comprehensive service plans incorporating personalized goals, interventions, and ongoing progress evaluation to enhance client outcomes.
• Provide individual and family counseling sessions that address anxiety, depression, grief, and adjustment issues related to hospice care.
• Empower clients through crisis intervention, risk assessments, and safety planning, offering immediate support during times of distress.
• Establish collaborative relationships with community agencies and resources, advocating for client needs and effectively coordinating services.
• Engage in proactive community outreach initiatives to illuminate our services and foster trust within the community, encouraging individuals to seek help.
• Participate in interdisciplinary team meetings and case conferences to optimize service delivery and promote unified care planning.
• Stay informed on best practices in the social work field, sharing insights with peers and participating in ongoing training sessions.

In this role, you will work primarily within a hospice setting, providing services to individuals and families approaching end-of-life transitions. Your empathy and expertise will be crucial in addressing their emotional and practical needs, helping clients cope with grief and loss while also providing support to their families. You will play an essential role in ensuring that each client receives compassionate care tailored to their unique life circumstances.

• A Master's degree in Social Work from an accredited institution is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Knowledge of trauma-informed care principles and social justice issues relevant to the populations served.
• Demonstrated expertise in case management practices and effective counseling methodologies.
• Strong communication skills, cultural competence, and the ability to solve complex problems effectively.

• Experience working within a social service agency, particularly in hospice or palliative care settings, is highly preferred.
• Familiarity with community resources, support groups, and specialized services related to end-of-life care.
• A background in mental health support and access to ongoing educational resources regarding grief and bereavement.
