Job description

The Craneย Safe Havenย 

Locatedย inย Manhattan,ย theย Craneย Safe Haven serves 100ย individualsย who areย not yet ready to transition into permanent housing, as well as those who may have struggled in other housing programs.ย Theย Craneย Safe Havenย offers an alternative to life on the street: private, safe, clean, and affordable short-term accommodations.ย On-site case management and clinical services are availableย toย meet the needs of itsย residentsย and to helpย those ready to move intoย secure permanent housing.ย 

Reporting to the Clinical Coordinator, the Case Manager, Transitional Housingย will provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.ย 

This position has a 35-hour work week.

ESSENTIAL DUTIES:

  • Provide direct services to a caseload of approximately 15 clients, including conducting necessary assessments, benefits assistance, and other services necessary to support the client in achieving housing and recovery goals
  • Conduct psychosocial evaluationsย 
  • Connect clients to psychiatric servicesย 
  • Connect clients to medical treatment and substance use treatment facilities when applicableย 
  • Obtain identification, public benefits, and other applicable income sources for clientsย 
  • Meet weekly with clients to assess needs, encourage harm reduction, and develop service plansย 
  • Prepare all documentation for housing applications
  • Assist and counsel clients in the process of obtaining permanent housingย 
  • Maintain record keeping in compliance with agency standards as well as those of City and State agenciesย 
  • Intervene in crisis situationsย 
  • Escort clients to appointments when neededย 
  • Upon training, provide assistance with self-administration of medication consistent with Breaking Ground's policies and procedures
  • Performs other related duties as assignedย 

MINIMUM QUALIFICATIONS:

  • Bachelorโ€™s Degree or equivalent work experience
  • Two years of experience working with homeless adults and substance abuse issues preferredย 
  • Experience with harm reduction and motivational interviewing techniques strongly preferred
  • Valid NY, NJ, CT, or PA driverโ€™s license with a good driving record, as defined by Breaking Ground is preferred
  • Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs
  • Excellent writing and communication skills
  • Bilingual Spanish/English is preferredย
