Unit based with caseload of 20-23 patients. Responsible for initial assessment within 24 hours, coordination of discharge plan and coordination of all post acute care needs. Will work with a variety of patients of varying ages and with various diagnoses. Will work closely with medical team, insurance case managers as well as Providence UM case managers to safely and quickly discharge patient to the lowest level of care possible. Special Equipment : - #Tier3 Travel Compliance We must have these three things before your traveler can be reviewed by our clinical team so please submit with this information included: What is their upcoming interview availability?.
