Work Environment

This full-time position is based onsite, primarily in a home care setting where you will interact directly with patients in their residences. Key Responsibilities

As a Physical Therapist at Revive Motion Clinic, your key responsibilities will include:

• Conducting comprehensive assessments to understand each patient’s unique health needs and disabilities.
• Developing and executing individualized treatment plans with specific, measurable goals.
• Providing ongoing evaluations and re-assessments to track patient progress and adjust treatment plans as necessary.
• Collaborating with physicians and other healthcare professionals to ensure continuity of care and comprehensive patient support.
• Educating patients and their families about conditions, treatment options, and preventive care strategies.
• Maintaining accurate and timely documentation of patient care activities and treatment outcomes.
• Supervising and mentoring physical therapy aides and students, ensuring a high standard of practice as they support your interventions.
• Participating in departmental leadership initiatives such as quality improvement, marketing strategies, and program development.

Patient Population / Client Focus

At Revive Motion Clinic, our patient population includes individuals recovering from orthopedic surgeries, those experiencing chronic pain, and patients with functional limitations due to injury or disease. We prioritize delivering compassionate care tailored to the unique needs of each patient, regardless of age or background. Your ability to establish rapport and effectively communicate will play a crucial role in patient adherence to treatment plans and overall recovery rates.

Qualifications

To succeed in this role, candidates must meet the following qualifications:

• A Bachelor’s Degree in Physical Therapy from an accredited institution.
• A valid state license in Physical Therapy.
• Current Basic Life Support (BLS) certification.
• Strong verbal and written communication skills for effective patient interaction and documentation.
• Proficiency in word processing and electronic health record (EHR) systems.

Preferred Experience

While not mandatory, the following experiences are considered advantageous:

• Previous experience working in a home care setting.
• Exposure to interdisciplinary team environments that enhance treatment methodologies.
• Knowledge of current practices in orthopedic rehabilitation.
