As a physical therapist working onsite, you will interact with a dynamic team of healthcare professionals, including Physical Therapy Assistants and an interdisciplinary team, to coordinate comprehensive patient care plans. As a Physical Therapist at Revive Motion Clinic, you will have a diverse range of responsibilities, including:

• Creating and implementing individualized physical therapy treatment plans aimed at patients' unique rehabilitation goals.
• Leading and supervising Physical Therapy Assistants in the delivery of care, ensuring adherence to treatment protocols.
• Conducting thorough evaluations utilizing diagnostic tests to assess clients' functional abilities and establish baselines for treatment.
• Assessing the home environment and making recommendations for modifications to facilitate patient independence.
• Providing in-home therapy services as prescribed by physicians, including administering manual therapeutic exercises and physical agents.
• Evaluating and recommending prosthetic and orthotic devices, ensuring optimal fit and function for each patient.
• Documenting clinical records meticulously, including initial evaluations, care plans, and progress notes, in compliance with all regulatory standards.
• Participating in interdisciplinary team meetings to collaboratively plan and evaluate overall patient treatment strategies.
• Educating and training patients and their families on effective use of therapy devices and overall rehabilitation programs.
• Completing all required documentation and orders promptly to meet clinical and administrative policies.

Your role will predominantly involve working with a varied patient population that includes individuals recovering from surgical procedures, those managing chronic pain, and patients in need of rehabilitation following injuries. By focusing on a personalized and empathetic approach, you will help patients regain their independence, lift their spirits, and achieve their rehabilitation goals.

The ideal candidate will meet the following criteria:

• Graduate of an accredited physical therapy program.
• A minimum of one year of experience as a physical therapist in acute care or home care settings.
• Current licensure as a Physical Therapist in the state of Texas, in good standing.
• Current CPR certification.
• A valid driver’s license and automobile insurance.
• Strong communication skills with an emphasis on consultative and collaborative approaches.
• A commitment to providing quality clinical care and support for patients and their families.

While not mandatory, applicants with a background in home healthcare and familiarity with the OASIS documentation process will be highly regarded. Your experience in coordinating care plans with interdisciplinary teams will set you apart, making you an invaluable asset to our clinic.
