This position is based onsite at our state-of-the-art rehabilitation facility in Sacramento. In this dynamic work environment, you will work collaboratively alongside a multidisciplinary team of healthcare professionals, ensuring seamless patient care. Working a standard 40-hour week, you will occasionally engage in weekend shifts to accommodate patient needs.

As a Physical Therapist, you will:

• Conduct comprehensive evaluations of patients, establishing individualized treatment plans based on functional limitations and disability assessments.
• Implement and monitor treatment programs, adjusting them based on patient progress and feedback.
• Provide clinical consultation to healthcare professionals involved in a patient’s continuum of care and coordinate interventional approaches.
• Document all treatment procedures accurately, ensuring compliance with professional standards and regulations, including timely submission of service charges.
• Engage with and educate patients and their families regarding treatment methodologies, outcomes, and post-discharge plans.
• Participate in community outreach programs to promote health and wellness initiatives, showcasing the clinic's commitment to service and care.
• Attend regular staff meetings, contribute to quality improvement projects, and participate in continuing education to sharpen your clinical skills.

The ideal candidate will possess the following qualifications:

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Current State License in Physical Therapy, demonstrating compliance with regulatory requirements.
• BLS (Basic Life Support) certification.
• Strong written and verbal communication skills, with proficiency in electronic health record systems.

Previous experience working in rehabilitation settings is preferred, especially those involving orthopedic care. Familiarity with evidence-based practices and a strong track record of contributing to patient outcome improvements will set you apart.
