Work EnvironmentThis position is primarily onsite where you will deliver therapy services directly within patients' homes throughout Jacksonville, Florida. The home care setting allows for a flexible and adaptive therapy environment, often leading to more significant engagement and better outcomes for patients. - Oversee clinical workflows that enhance patient capture rates, as well as the processing of referrals and scheduling appointments to streamline service delivery.
- Implement comprehensive therapy plans for each patient, emphasizing the importance of early intervention and conducting same-day evaluations to facilitate prompt care.
- Establish a professional and collaborative working atmosphere that cultivates teamwork, quality service, and patient safety through active colleague participation.
- Collaborate with the Director of Therapy Operations (DTO) to identify and develop clinical improvement opportunities, contributing to the overall excellence of our practice.
- Nurture strong relationships with local employers, referral sources, and payer groups through consistent communication and engagement initiatives to foster a network of support for our patients.
You will primarily serve patients who are experiencing challenges due to temporary or permanent disabilities, postoperative recovery, or chronic conditions affecting mobility. Your expertise will be vital in assessing their unique needs, developing personalized treatment plans, and actively engaging with other healthcare professionals to provide holistic care that enhances patient mobility and independence.
Qualifications- Possession of an active Florida State license to practice as a Physical Therapist.
- A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Strong communication, interpersonal, and organizational skills that promote effective patient interactions and team collaboration.
- Capability to work efficiently in a fast-paced, dynamic environment, with a commitment to excellence in patient care.
- Demonstrated problem-solving abilities and critical thinking skills to navigate complex clinical scenarios.
- A genuine passion for improving patient outcomes and enhancing their overall quality of life through rehabilitative therapy.
While not mandatory, candidates with previous experience in home health environments or those who have worked with diverse patient populations will be viewed favorably. Familiarity with electronic health records (EHR) and telehealth systems is beneficial, as these tools play a key role in tracking patient progress and maintaining communication within the care team.
Compensation and Benefits- USD 120,000 to 130,000, commensurate with experience and caseload management.
