• Conduct comprehensive assessments of clients experiencing symptoms of major mental illness and severe emotional disorders.
• Develop and implement individualized integrated care plans that encompass preventive, therapeutic, and recovery services.
• Collaborate with a diverse interdisciplinary team to enhance patient outcomes and streamline access to essential resources.
• Assist in crisis interventions, ensuring that clients receive immediate support as needed.
• Monitor client progress through regular meetings, reviewing admission criteria, and assessing ongoing treatment needs.
• Participate actively in team evaluations and quality improvement committees to enhance service delivery across our organization.
• Maintain accurate and detailed records of client interactions and treatment progress, ensuring compliance with ethical standards and regulatory requirements.
• Provide support in determining discharge criteria and follow-up care, working closely with clients and their families.
• Engage in professional development opportunities and training sessions to stay current with best practices in social work.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
• Strong knowledge of mental health diagnosis, risk assessment, crisis intervention, and treatment methodologies.
• Excellent verbal and written communication skills, with the ability to build rapport with clients and team members.
• Demonstrated cultural competence and sensitivity towards diverse populations.
• Proficient organizational and planning skills, capable of managing a caseload efficiently.

• A minimum of 2 years of clinical experience working with similar populations is preferred.
• Experience utilizing evidence-based practices and integrated care approaches in therapeutic settings.
• Familiarity with electronic health record (EHR) systems and data management tools.
