Job description

The Medical Records Clerk supports the processing and release of medical records while ensuring requests are handled accurately, securely, and in accordance with applicable privacy laws, policies, and procedures. This role also provides administrative support related to medical records and policy tracking. You will work remotely and provide administrative support related to medical records and policy tracking and will report to the Senior Compliance Professional.Regularly exercises discretion and judgment in prioritizing requests and interpreting and adapting procedures, processes and techniques, and works under limited guidance due to previous experience/breadth and depth of knowledge of administrative processes and organizational knowledge.

As a Medical Records Clerk, you will support medical record requests from patients, government agencies including subpoenas, healthcare providers, and other authorized third parties. Your responsibilities will include:

  • Review, and respond to patient, government, and other third-party requests for the release of medical information.
  • Ensure medical record requests are received and triaged through established departmental workflows.
  • Process requests following applicable state and federal privacy laws, organizational policies, procedures, and departmental guidelines.
  • Counsel and assist requesters with the medical record request process, including explaining requirements, estimated completion times, provider responsibilities, and other factors that may affect the request.
  • Identify the appropriate type of release request and complete or route the request to the appropriate department.
  • Recognize protected health information (PHI) requests that may require additional review or action and follow established procedures.
  • Validate authorizations and medical record requests using established checklists and guidelines to ensure all required information is complete.
  • Securely manage and release medical information through approved methods, including electronic systems, email, fax, or mail.
  • Review medical record requests for completeness and obtain additional information, documentation, authorizations, or signatures when needed.
  • Maintain detailed documentation and notes to track each request from receipt through completion.
  • Create reports to monitor medical record request volumes and complete requests within established turnaround times.
  • Extract information from internal systems to complete medical record requests.
  • Maintain patient confidentiality and protect sensitive medical and health information.
  • Help with the implementation and maintenance of medical records processes that meet professional standards and applicable federal, state, and local requirements.
  • Track policy updates and annual policy review dates.
  • Provide general administrative support related to organizational policies, procedures, medical records, and policy tracking.
  • Complete required annual training and remain current on applicable policies, procedures, and state and federal privacy laws.

Preferred Qualifications

  • One year of experience working in a healthcare setting.
  • Experience reading and understanding clinical or hospital health records.
  • Previous medical records or release-of-information experience.
  • Experience working with electronic medical record (EMR/EHR) systems.
  • Familiarity with HIPAA and healthcare privacy requirements.
  • Experience with Adobe Acrobat Professional or similar software used to combine, organize, bookmark, redact, and password-protect medical record files.
  • Associate's or bachelor's degree in healthcare administration, health information management, or a related field.
Work at Home Requirements: To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
