This position is designed to be performed remotely, allowing for flexibility while still prioritizing patient care. While working from home, the LPC will be part of a collaborative outpatient office setting where teamwork and communication with primary care providers are essential. You will have access to advanced telehealth tools and case management systems that facilitate ongoing patient engagement and treatment planning.

• Conduct thorough patient intakes, performing diagnostic and functional evaluations to develop personalized treatment plans.
• Provide brief, focused interventions to clients, tailoring your approach to meet individual needs based on established guidelines.
• Collaborate with primary care providers to identify patients requiring additional support and determine appropriate referral pathways to specialty mental health services.
• Monitor patient progress, ensuring regular follow-ups to evaluate treatment effectiveness and adapt strategies as necessary.
• Develop and implement relapse prevention plans, assisting clients in maintaining stability and managing mental health challenges.
• Document patient interactions meticulously in clinical records, maintaining confidentiality while actively contributing to their care history.
• Conduct educational sessions to empower patients with self-management skills, enabling them to participate fully in their treatment and recovery processes.
• Engage in interdisciplinary teamwork, sharing insights and contributing to clinic management to enhance service delivery and program evaluation.
• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals while carrying out additional duties as assigned.

• A Master’s Degree in Clinical Counseling.
• Current state-issued LPC (Licensed Professional Counselor) License.
• In-depth knowledge of mental health assessment, treatment planning, behavioral therapies, and the DSM-IV diagnostic criteria.
• Sound understanding of the psychosocial impact of various health issues on individuals and their families.
• Solid clinical judgment and the ability to make independent decisions based on professional insights.
• Excellent communication skills, with the ability to establish positive working relationships with colleagues and patients alike.
• Competency in maintaining detailed clinical records and preparing reports.

• Bi-lingual proficiency in English and Spanish is preferred but not mandatory.
• Prior experience in an outpatient mental health setting is beneficial.
• Familiarity with community resources and networks that support mental health initiatives.

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
