Job description

About Hopscotch Primary Care
At Hopscotch Primary Care, we believe great healthcare should be accessible to all people across all communities. Today, almost 20% of Americans live in a rural community, yet only 11% of physicians practice in those same communities. We are on a mission to transform healthcare in rural America. We provide high-quality primary care tailored to meet the needs of our patients through our robust care model and comprehensive care team, delivering care in our clinics, and across settings, and wrapping resources around the patients who need them most.
Our patients and the care teams who serve them sit at the center of everything we do at Hopscotch. Hopscotch Primary Care takes a team approach to serve patient needs and provide the best care possible. Our goal is to provide the care each of us would want for ourselves or for our family members, in the right setting, and at the right time.
Today, we are serving thousands of patients in our value-based care model and the number is growing every day. If you want to bring your experience, skill and passion to make a lasting impact in healthcare, we'd like to meet you.
ABOUT THE ROLE
At Hopscotch Primary Care, we take a team approach to serve patients' needs and provide the best care possible. Our goal is to deliver the care each of us would want for ourselves or our family members, in the right setting and at the right time.
As an Outbound Sales Representative, you will play a key role in supporting patients during the Annual Enrollment Period (AEP). You will connect with patients by phone, educate them about their healthcare coverage options for the upcoming year, and ensure they feel confident and supported in navigating their choices. In this role, you will work closely with the center team members and insurance brokers to make sure each patient's needs are fully addressed and that they are set up for success in 2027.
WHAT YOU'LL DO
Specific responsibilities for this role will include, but are not limited to:
Patient Engagement & AEP Support
  • Conduct outbound and inbound calls with patients during the Annual Enrollment Period to review coverage options and answer questions
  • Partner with PRMs and brokers to coordinate support, ensuring patients receive the guidance they need
  • Assist patients in navigating their healthcare coverage options with patience, and clear communication
  • Document all patient interactions, next steps, and outcomes in relevant systems

Collaboration & Coordination
  • Work hand-in-hand with the center team members to identify patients needing extra support and follow through on action items
  • Coordinate with insurance brokers to ensure patients' insurance transitions are smooth and all questions are addressed
  • Provide regular updates to the center team members to maintain alignment on patient needs and enrollment progress
  • Share patient feedback and barriers with leadership to improve processes and patient experience

Sales & Outreach
  • Execute a structured outreach cadence (calls, texts, emails) to ensure timely follow-up with patients during AEP
  • Manage a high volume of daily outreach while maintaining high-quality, personalized communication
  • Track performance metrics and contribute to enrollment success targets

ABOUT YOU
You would be a great fit for this position if you are motivated by helping patients make confident insurance decisions, enjoy communicating over the phone, and thrive in a collaborative, team-focused environment
A great candidate for this role:
  • Has demonstrated success in sales, inside sales, or customer service (healthcare or insurance preferred)
  • Possesses 1-2 years of experience in a patient-facing or customer-facing role
  • Is comfortable managing a high volume of calls and follow-ups during peak enrollment periods
  • Is empathetic, a strong communicator, and an active listener
  • Is organized, detail-oriented, and proactive in coordinating across multiple stakeholders
  • Builds trust quickly and can handle sensitive conversations with professionalism
  • Enjoys talking with seniors
  • Is comfortable using email, Microsoft Office applications, and computer reporting tools
  • Is collaborative and works well with center team members, brokers, and other internal partners
  • Has U.S. work authorization (required)

Preferred but not required:
  • Prior healthcare, insurance, or senior services experience
  • Familiarity with Medicare Advantage, Dual-Eligible populations, or enrollment processes

Additional skills and capabilities required:
  • Positive and solutions-oriented mindset
  • Ability to simplify complex information for patients in a clear, supportive way
  • Strong organizational skills and ability to manage multiple priorities simultaneously

At Hopscotch Primary Care, we embrace diversity, invest in a culture of inclusion and positivity and encourage all to apply to join our team. You will receive consideration for employment without regard to race, color, religion, gender, gender identity or expression, sexual orientation, national origin, genetics, disability, age, or veteran status.
