A Brief Overview
Supervises social workers (clinical and non-clinical), care coordinators, and licensed professional counselors in DCAPBS. Responsible for clinical supervision of new patient intake and access, and care coordination activities in the DCAPBS Outpatient Program. Assures that intake and patient access triage are consistent with clinical decision making guidelines established by the Outpatient Program leadership team. Works collaboratively with DCAPBS Outpatient Director, Medical Director, Associate Director, and Practice Manager to assure the highest quality patient care within a health, collaborative, efficient and accountable environment. Coordinates and manages all levels of social work training within DCAPBS. Engages in professional clinical service delivery as a licensed clinical social worker.
What you will do
- Management of Licensed Clinical Social Workers (LCSW) and Licensed Professional Counselors (LPC)
- Collaborates with medical and clinical directors of DCAPBS programs in which LCSW/LPC clinicians provide patient care regarding patient care practices, productivity, schedule availability, and adherence to patient care policies.
- Provides clinical guidance and supervision to LCSW/LPC clinicians. Leads the recruitment of LCSW and LPC clinicians.
- Participates in the annual performance of LCSW/LPC clinicians
- Collaborates on performance evaluations with other clinical program and operations leaders.
- Provides professional development guidance and mentorship to DCAPBS LCSW/LPC clinicians.
- Facilitate and ensures completion of FPPE and OPPE for all DCAPBS LCSW/LPC clinicians; assures that DCAPBS LCSW/LPC clinicians maintain discipline-specific professional competencies.
- Maintains relationships with CHOP Social Work department to ensure that all DCAPBS social workers and LPC's are integrated within CHOP Social Work (e.g. onboarding and orientation, training opportunities, mandatory education, career development guidance).
- Management of Intake Coordinators and Care Coordinators:
- Provides clinical supervision to Outpatient Program case managers and intake coordinators (clinical management, day-to-day clinical decision making related to clinical information gathering and scheduling).
- Collaborates with DCAPBS Outpatient Program Associate Director and Practice Manager on oversight of intake and scheduling processes.
- Responsibility for providing guidance related to clinical decision making regarding scheduling and assisting patients with accessing patient care resources (both internal and external to the CHOP Care Network).
- Supports Outpatient Program Practice Manager in oversight of intake and care coordination personnel.
- Works with Outpatient Program leadership (primarily with Outpatient Program Associate Director) to assure that intake decision-making follows triage guidelines established by Program leadership, including training intake coordinators to apply triage guidelines and reviewing cases for quality assurance. Provides supervision to intake coordinators to support triage guidelines being followed with fidelity.
- Supports intake coordinators and care coordinators in maintaining knowledge of clinical services available within DCAPBS, the CHOP Care Network, and other programs connected with the CHOP enterprise, as well as the community-at-large.
- Works in partnership with Outpatient Program Practice Manager on recruitment of intake coordinators, case managers, care coordinators.
- Participates in the annual performance evaluations of intake coordinators, case managers, and care coordinators to provide objective feedback on specific actions related to job performance and the impact of job performance on operations.
- Provides clinical guidance and supervision, professional development guidance and mentorship to DCAPBS Outpatient Program intake coordinators, case managers, care coordinators (including development of more advanced care coordination skills to support both new and established patients)
- Ensures that DCAPBS intake and care coordination social workers are integrated within the CHOP Social Work Department.
- Applies the CHOP Leadership Competencies of Accountability, Care and Service, Change Leadership, Diversity, and Effective Management in duties related to supervision and management of intake and care coordination, as well as the CHOP ICARE Values (Integrity, Compassion, Accountability, Respect, and Excellence).
- Professional Practice
- Assesses, plans, implements professional interventions based upon knowledge of social work practice, human behavior and the social environment, as well as child and family development and functioning. Interventions are directed to the required by patients and their families around illness and disease, hospitalization, and any continuing care that may be required over time.
- Performs psychosocial assessment with patients and/or family.
- Intervenes/treats patient and/or family-based on the psychosocial assessment providing age and developmentally appropriate counseling, treatment and resources as indicated.
- Encourages and utilizes feedback from patients, families and other staff to improve professional services. Evaluates the effectiveness of professional services and redefines goals or modalities as indicated. Organizes individual workload and sets priorities based on patient care needs and departmental standards. Meets workload standards for assigned areas.
- Adheres to professional medical - legal reporting requirements. Seeks appropriate supervision on risk management issues.
- Responsible for the development and enhancement of professional practice skills.
- Recognizes areas of strength and areas for development. Develops plan for supervision, consultation and/or learning.
- Utilizes team members, peers and supervisory staff in consultation for professional development.
- Incorporates new knowledge and feedback into practice.
- Meets annual requirements for professional continuing education.
- Meets annual requirements for mandatory education.
- Subscribes and adheres to the NASW Code of Ethics
- Responsible for accurate documentation of professional services in the patient's electronic medical record. All requirements for record keeping and documentation, including billing data, are to be completed in a timely manner.
- Required documentation of assessments and interventions are well organized, clear, and timely.
- All written work is compliant and recorded properly according to hospital policy.
- Required information system data is completed and submitted according to program, department and hospital standards.
- Completes hospital, department and program requests for information within required time frames.
- Completes and submits any required reports or forms to other departments or outside agencies (as appropriate) within required time frames..

Education Qualifications
