About the Role:
Life Consultants Inc. is seeking a highly motivated and compassionate Human Service Professional to join our team in Chesapeake, VA. As a Human Service Professional, you will assist in supporting individuals who are in transitional housing and or in a crisis home who struggle with severe mental health diagnosis and they typically need support locating permanent housing.
Responsibilities:
  • Provide direct support and counseling to individuals facing various challenges
  • Assist clients in setting and achieving personal goals
  • Develop and implement individualized treatment plans
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Advocate for clients' rights and access to resources
  • Maintain accurate and up-to-date documentation of client progress
  • Stay informed about current best practices and trends in human services
  • Provide supportive counseling/crisis intervention services to individuals as needed.
  • Rotating between company facilities to include transitional housing and corporate office.
  • Transport client(s) to appointments and etc.

Requirements:
  • Minimum of 2 years of experience in a similar role or degree in Human Services or related field
  • Valid state licensure or certification as a Human Service Professional
  • Strong interpersonal and communication skills
  • Ability to work effectively in a team and independently
  • Knowledge of local resources and support services
  • Empathy and compassion for individuals facing challenges
  • Proficiency in documentation and record keeping
  • Applicants must be able to pass background checks.
