Job description

About the Role
We are seeking an experienced Intensive Case Manager to support formerly homeless adults residing in Permanent Supportive Housing. This is a fully onsite position focused on helping residents maintain housing stability through ongoing case management, crisis intervention, resource coordination, and connection to community services.
Responsibilities
  • Manage a caseload of residents in Permanent Supportive Housing.
  • Conduct regular resident check-ins and assessments.
  • Connect residents to mental health, healthcare, benefits, employment, and community resources.
  • Coordinate services with behavioral health providers and property management.
  • Provide crisis intervention, de-escalation, and housing retention support.
  • Maintain accurate case notes, service plans, and program documentation.
  • Participate in team meetings, trainings, and collaborative care planning.
Qualifications
  • Minimum 2 years of case management experience working with homeless or vulnerable populations.
  • Experience supporting individuals with mental health, substance use, or co-occurring disorders.
  • Knowledge of Housing First, trauma-informed care, motivational interviewing, and harm reduction practices.
  • Strong communication, documentation, and problem-solving skills.
  • Ability to work independently in a fast-paced supportive housing environment.
Preferred
  • Experience in ICMS, PSH, homeless services, behavioral health, or supportive housing programs.
  • Familiarity with HMIS or other case management systems.
  • Bachelor's degree in Social Work, Human Services, Psychology, or a related field.
Onsite in Pomona, CA | 3+ month contract covering for LOA
Pay Details: $24.00 to $28.00 per hour
