Summary:ย 


The Case Manager plays a key role in fulfilling the mission of St. Vincent de Paul of Alameda County by providing compassionate, person-centered support to individuals experiencing homelessness as they work toward stable, permanent housing. Working directly with guests in our emergency shelter, the Case Manager conducts case management, housing navigation,ย and resource coordination while helping remove barriers to housing stability. This role serves a diverse population, including individuals experiencing mental health challenges, substance use disorders, and other complex needs, and approaches every interaction with dignity, respect, and the belief that everyone deserves the opportunity for a fresh start.ย

DUTIES & RESPONSIBILITIES:ย ย 


  • Provide case management, housing navigation,ย resource referrals, and service linkages for all shelter guests.ย 
  • Develop individualized service plans that address housing, employment, physical health, behavioral health, substance use recovery, food security, and other basic needs.ย 
  • Prioritize intensive case management services for guests who are not currently connected to an existing case management program.ย 
  • Assist guests in securing permanent and transitional housing through hands-on housing navigation and advocacy.ย 
  • Establish and maintain relationships with landlords, property managers, housing providers, housing agencies, and other community partners to expand housing opportunities.ย 
  • Connect guests with Oakland Housing Resource Centers and other community-based housing resources.ย 
  • Coordinate referrals and ongoing services with medical, behavioral health, substance use treatment, domestic violence, employment, and other supportive service providers.ย 
  • Provide one-on-one support, coaching, advocacy, crisis intervention, conflict resolution, and peer support to help guests achieve successful outcomes.ย 
  • Conduct Coordinated Entry Assessments utilizing Alameda County's Coordinated Entry System upon completion of required training.ย 
  • Reconnect guests with existing case management providers whenever appropriate to ensure continuity of care.ย 
  • Maintain accurate and timely documentation within the Homeless Management Information System (HMIS) while ensuring compliance with HIPAA and confidentiality requirements.ย 
  • Track client progress, housing outcomes, referrals, service linkages, grant-related metrics, and other program data.ย 
  • Prepare and submit accurate monthly reports documenting case management activities and program outcomes for organizational leadership and the City of Oakland.ย 
  • Maintain detailed records required for grant compliance, contract reporting, and program evaluation.ย 
  • Serve as a Mandatory Reporter in accordance with state and organizational requirements (training provided as needed).ย 
  • Represent Saint Vincent de Paul of Alameda County at community meetings, collaborative partnerships, outreach events, and other external activities.ย 
  • Participate in ongoing professional development, cross-training, and staff training opportunities.ย 
  • Perform other duties as assigned in support of the mission of Saint Vincent de Paul of Alameda County.ย 

Requirements

GENERAL QUALIFICATIONS: ย 

  • Demonstrated experience in case management, housing navigation, client advocacy, crisis intervention, and resource coordination.ย 
  • Knowledge of trauma-informed care, Harm Reduction, Motivational Interviewing, and OARS principles.ย 
  • Experience working with the Homeless Management Information System (HMIS) or similar case management database.ย 
  • Strong understanding of housing resources, supportive services, and community-based organizations throughout Alameda County.ย 
  • Ability to build trusting relationships with individuals from diverse backgrounds, including LGBTQ+ communities, youth, immigrants, low-income individuals, and communities of color.ย 
  • Understanding of the impact of trauma, domestic violence, mental health challenges, and substance use disorders on housing stability.ย 
  • Excellent interpersonal, verbal, and written communication skills, with fluency in English required.ย 
  • Strong critical thinking, problem-solving, and conflict resolution skills, with the ability to remain calm and effectively de-escalate crisis situations.ย 
  • Proficiency in Microsoft Office Suite, including Word and Excel, with accurate data entry and reporting skills.ย 
  • Ability to maintain accurate documentation, prepare reports, and manage confidential information in compliance with HIPAA and organizational policies.ย 
  • Ability to establish and maintain effective working relationships with clients, colleagues, volunteers, community partners, and external stakeholders.ย 
  • Demonstrated ability to maintain appropriate professional boundaries while providing compassionate, person-centered services.ย 
  • Must be able to work independently, prioritize multiple responsibilities, and adapt in a fast-paced, team-oriented environment.ย 
  • Must be willing to become a Mandated Reporter (training provided, if necessary).ย 
  • Demonstrated commitment to the mission and values of the Society of St. Vincent de Paul of Alameda County.ย 

SPECIFIC QUALIFICATIONS:ย 

  • Minimum of one (1) year of experience providing direct services to individuals experiencing homelessness, survivors of domestic violence, or other vulnerable populations.ย 
  • Proficiency with Windows, Word, Excel, e-mail, and Web Browser.ย ย ย 
  • Excellent time and priority management skills. Ability to successfully manage projects from conception to completion. Ability to prioritize projects, work independently, meet deadlines, manage several projects simultaneously and work well under pressure.ย ย ย 
  • Ability to understand the financial impact of decisions, actions, and outcomes.ย ย ย 
  • Project management skills, including the ability to create and follow outlines, timelines, and reports, and to hold others accountable for completion of tasks.ย ย 
  • Supervisory or management experience.ย ย 
  • Good analytic skills and ability to make evidence-based decisions.ย ย 
  • Ability to work well with diverse groups of people in various environments, including those that are homeless, addressing substance abuse issues and other barriers to employment, people from diverse ethnic, cultural and economic backgrounds, the undocumented, and people with disabilities including HIV/AIDS.ย ย ย 

EDUCATIONAL REQUIREMENTS:ย 

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred; equivalent combination of education and relevant experience will be considered.ย
