Our client is a rapidly growing, pre-Series A surgical navigation company that is completely redefining how self-insured employers manage healthcare spending. As the worldโ€™s first fully transparent, medically led Centers of Excellence platform, they partner with top-tier global medical providers to deliver massive cost savings on high-cost specialty care.

They are looking for a sharp, tech-savvy Patient Care Coordinator (LVN) to join their tight-knit, high-performing team. In this role, you won't just be managing careโ€”youโ€™ll be the first point of contact for patients, guiding them through enrollment, and utilizing cutting-edge AI tools to scale impact. If you have a healthy dose of business acumen, thrive on direct feedback, and genuinely get excited about technology, this is your sandbox.


What You Will Be Doing

  • Patient Engagement: Making 50โ€“75 outbound cold calls per week to actively engage and enroll eligible patients into their surgical benefits.

  • Caseload Management: Guiding patients seamlessly through onboarding, verifying eligibility, and collecting necessary medical records.

  • Care Coordination: Managing flawless handoffs between patients, nurses, providers, and the internal operations team.

  • AI & Process Innovation: Collaborating on AI scripting and workflow enhancements by sharing direct insights from your daily patient interactions.

  • Executive Collaboration: Working directly alongside the executive leadership team to optimize operational improvements and drive key business metrics.


Key Requirements

  • Licensure: Active LVN license (any stateโ€”this is a non-clinical, care navigation role).

  • Experience: 2โ€“7 years of experience in remote healthcare care coordination, patient engagement, or case management.

  • Startup Savvy: Prior experience within a healthcare startup or health tech environment, demonstrating strong business acumen and a comfort level with direct, candid feedback.

  • Tech-Forward: Highly tech-savvy and genuinely excited about AI tools; you view rapidly evolving technology as an asset, not a threat.

  • Location & Schedule: Must be based in the SF/Bay Area (or willing to commute from as far as Sacramento) to spend 6 days per month in the San Francisco office. The remaining 70% of the time is remote work-from-home.
