Midland Care Connection

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

PACE Program 

The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

Transportation Specialist

Ensures that PACE participants get where they need to go safely, comfortably, and on time. This role involves driving buses, vans, or cars to transport participants between their homes, medical appointments, and the PACE Day Center while observing safety regulations and using proper transfer techniques. You’ll assist participants in and out of vehicles and buildings, help with wheelchairs and ambulatory transfers when needed, and maintain a supportive, respectful environment during trips. Transportation Specialists also help keep vehicles clean and in good working order, perform basic safety checks, and communicate any schedule changes, participant concerns, or vehicle issues to the care team. In some positions, you may also observe and report changes in participants’ health or behavior and support light duties at the Day Center as needed.

Required

  • Valid Kansas or Missouri driver’s license (or ability to obtain one within a specified time frame)
  • Current automobile liability insurance
  • Motor vehicle record that meets organizational and insurability standards
  • Reliable transportation to and from work
  • Ability to pass and maintain a Department of Transportation (DOT) physical and drug screening
  • Must be 25 years of age or older (insurance requirement)
  • Physical ability to assist participants, including bending, lifting, and helping with wheelchair or ambulatory transfers

Preferred

  • Prior professional driving experience
  • Experience working with frail, elderly, or disabled individuals
