Full-Time Hospice On-Call Licensed Vocational Nurse (LVN)

Location: Various service areas across California LA County Riverside County San Bernardino County Orange County

Schedule: Full-Time On-Call Tuesday, Wednesday, Thursday 4:30 pm to 9 am Friday 4:30 pm to 9 am Sunday

Preferred* for Person to reside in or around: Central Area for easy access to above locations such as Whittier

Position Summary

The Full-Time Hospice On-Call LVN is responsible for responding to urgent patient needs during on-call hours, providing symptom management, supportive care, and timely communication with the RN Case Manager and interdisciplinary team. This role is ideal for an LVN who is confident, calm under pressure, and able to work independently in home settings.

Key Responsibilities

Respond promptly to urgent calls, including patient/family concerns, symptom changes, equipment needs, or caregiver support.

Perform after-hours visits for assessment, comfort care, and patient needs within LVN scope of practice.

Communicate patient condition changes to the RN Case Manager or on-call supervisor.

Assist with PRN visits such as medication administration, routine care, and safety checks.

Provide emotional support and education to caregivers regarding comfort-focused care.

Complete supervisory visits for hospice aides when assigned.

Document all care accurately and timely according to Medicare, state, and agency standards.

Participate in team communication to support continuity of care.

Qualifications

Active California LVN license in good standing.

1+ year hospice or home health experience preferred.

Strong assessment skills and ability to work independently.

Reliable transportation, valid CA driver's license, and auto insurance.

Ability to remain calm, compassionate, and professional during urgent or stressful situations.

EMR experience preferred (training provided).

Reliable availability during assigned on-call hours.
