Job description

WAGE RATE - $75K - $85K/Year

GENERAL DESCRIPTION:

MDS (Minimum Data Set) nurse collects and assesses information for the health and well-being resident in Medicare or Medicaid-certified beds.ย  Complies with Federal and State requirements for all MDS sections.ย  Follows facility policy and procedures.ย  The MDS Nurse monitors residentsโ€™ health and well-being from the time of admission through the end of their stay.ย  Assist with Quality Assurance and quality measures.

RESPONSIBLE TO: Administrator

DUTIES ANDย RESPONSIBILITIES: Include, but are not limited to, the following.

1. Completes the nursing components of the Resident Assessment Instrument (RAI) process to ensure timely completion of Minimum Data Set.

2. Ensures completion of PPS and/or OBRA assessments for both subacute and long-term care as required.

3. The MDS Nurse utilizes State and Federal guidelines for the RAI process ensuring documentation meets necessary guidelines and reimbursement standards.

4. Gathers information from nursing, dietary, social services, therapy, and physician services to ensure appropriate quality of care and reimbursement.

5. MDS Nurse will assist with completing nursing assessments.

6. Ensure legal compliance for the facility by following the MDS Process.

7. Assists with determination coverage for skilled Nursing services and provide. complete team with documentation information to prevent payment denials.

8. Utilizes the RAI, MDS 3.0, QM and QRP manuals to ensure the highest quality of care is provided, documentation supports outcomes and maximum reimbursement is achieved.

9. Review 24-hour report for updates and possible change of condition.

10. Assists with the Quality Assurance Program as needed.

11. Participate in daily review of new admission medical record, MD orders and nursing reports

12. Ensure compliance with regulations utilizing the MDS schedule to plan and set MDS ARD

13. Maintains and develops on-going MDS schedule

14. Audit and analyze MDS data for accuracy

15. Works in collaboration with the Unit Managers and unit staff for educational purposes and accuracy of documentation

16. Educates members of the IDT as necessary in completion of the MDS and any changes in the MDS process

17. Attends weekly Medicare meeting to ensure that eligible residents receive Medicare coverage as appropriate and ensures appropriate documentation is in medical record

18. Works with residents, families as needed regarding questions on Medicare benefits and related issues (demand bill)

19. Issues NOMNC to residents/responsible party and assists with the appeals process, as needed.

20. Completes QA audits as assigned

21. Recognizes link between MDS data and Quality Improvement measures.

22. Maintains open communication with Medical Staff, Administrator, DON, ADON and Pharmacy consultant

23. Attends Nurse Management meetings, Patient Care meetings and any other meetings as assigned

24. Performs other tasks as assigned

Must be able to work more than eight (8) hours per day or more than forty (40) hours per week.

QUALIFICATIONS:

1. Current NYS Licensed Registered Nurse

2. Knowledge of MDS 3.0, CAAโ€™s, Medicare and Medicaid Guidelines for coverage and reimbursement

3. RAC-CT Certification preferred

4. Excellent written and oral communication skills with the ability to work independently and under time constraints

5. Long-Term Care Experience

6. Basic computer skills
