We are seeking an experienced MDS Director to lead a highly stable and well-supported MDS department at one of our trusted partner facilities. This is an opportunity to step into a thriving environment where you will be valued, supported, and set up for success.

You will be joining a stable MDS department with two long-tenured, highly skilled MDS Coordinators who are collaborative and team-oriented. You will work alongside a supportive Director of Nursing and Administrator who value partnership and leadership. You will experience a positive, professional culture where leadership is engaged and focused on helping the team succeed. The MDS department is well-run and stable; this is not a rebuild situation. The facility is proud of its staff longevity and strong culture of teamwork and trust.

Your Role:

Lead and oversee the MDS program, ensuring accuracy and compliance. Optimize PDPM processes and reimbursement opportunities. Provide leadership, mentoring, and ongoing support to MDS Coordinators. Collaborate with nursing and interdisciplinary teams for comprehensive care planning. Promote a culture of continuous learning, accountability, and innovation.

What We Are Looking For:

RN license preferred. Proven experience in long-term care and MDS coordination. In-depth knowledge of MDS 3.0, PDPM, Medicaid/Medicare, and EMR systems. Strong leadership and communication skills. A collaborative, team-first mindset and dedication to resident care.
