Location: Port Charlotte, Florida
Schedule: Full-Time | Mondayโ€“Friday | 8:00 AM โ€“ 5:00 PM
Pay: $46โ€“$49 per hour, based on experience

Join Our Team as an MDS Director!

Charlotte Bay Rehab and Care Center is seeking an experienced and compassionate Registered Nurse (RN) to lead our MDS and Care Planning program. This leadership role is essential in ensuring accurate resident assessments, regulatory compliance, quality outcomes, and person-centered care.

Key Responsibilities
  • Oversee the MDS assessment process to ensure accuracy, timeliness, and regulatory compliance.

  • Conduct comprehensive assessments for new admissions and current residents.

  • Develop, update, and maintain individualized care plans that support each resident's clinical goals.

  • Lead interdisciplinary care plan meetings with physicians, nursing staff, therapy, residents, and families.

  • Ensure compliance with Medicare, Medicaid, and state and federal regulations related to MDS and reimbursement.

  • Monitor MDS schedules and maintain compliance with assessment deadlines for Medicare and non-Medicare residents.

  • Collaborate with clinical leadership to improve quality measures and resident outcomes.

  • Educate and mentor nursing staff on MDS, RAI guidelines, RAPS, Care Area Assessments (CAAs), and regulatory updates.

  • Review documentation to ensure accuracy and support appropriate reimbursement.

  • Participate in quality assurance and performance improvement (QAPI) initiatives.

Qualifications
  • Current Florida Registered Nurse (RN) license in good standing.

  • Minimum of two (2) years of experience in a Skilled Nursing Facility.

  • Previous MDS Coordinator or MDS Director experience preferred.

  • Strong working knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and care planning.

  • Excellent leadership, communication, organizational, and time management skills.

  • Ability to work collaboratively with an interdisciplinary team.
