Facility: Platte Care Center Avera
Facility Type: LTC / Nursing Home
Location: Platte, SD
Duration: 12–14 Weeks
Shift: Day Shift (8:00 AM – 4:30 PM)
Hours: 32–36 Hours/Week
Pay Rate: $52 – $54/hr
Estimated Weekly Pay: $1,664 – $1,944/week (32–36 hrs)
Charting System: EPIC


Job Summary

The RN – MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments for residents in a long-term care setting. This role ensures accurate resident assessments, compliance with regulatory requirements, and proper documentation to support care planning and reimbursement processes. The coordinator works closely with interdisciplinary teams to maintain quality outcomes and resident-centered care.


Requirements
  • Active RN license (Required)

  • Experience in LTC / Skilled Nursing Facility preferred

  • MDS coordination or MDS coding experience preferred

  • Strong understanding of CMS regulations and care planning processes

  • Ability to complete accurate and timely resident assessments

  • Strong documentation and communication skills

  • Familiarity with EPIC charting system preferred


Required Credentials
  • Active Registered Nurse (RN) License

  • BLS Certification (Preferred depending on facility policy)

  • 50-bed Skilled Nursing Facility

  • EPIC EMR System

  • Break: 30 minutes per shift

  • Scrub Policy: Any scrubs allowed except all-black sets (top and bottom together not permitted)
