As an RN MDS Coordinator, you'll play a vital role in ensuring accurate assessments, care planning, reimbursement optimization, and regulatory compliance while collaborating with an interdisciplinary team dedicated to excellence.

RN MDS Coordinator Qualifications
  • Active, unencumbered New Jersey Registered Nurse (RN) license

  • Associate's or Bachelor's Degree in Nursing

  • Minimum of two (2) years of experience as an RN MDS Coordinator in a long-term care setting

  • Previous skilled nursing or long-term care experience preferred

  • Strong understanding of MDS regulations, reimbursement systems, and care planning processes

  • Excellent communication, organizational, and leadership skills

RN MDS Coordinator Responsibilities
  • Conduct and coordinate Minimum Data Set (MDS) assessments in accordance with federal and state regulations

  • Support individualized care planning and participate in care plan meetings

  • Monitor resident progress and update assessments to reflect changes in condition

  • Work closely with nursing and administrative teams to support accurate billing and reimbursement processes

  • Provide education and guidance to staff regarding documentation requirements and assessment procedures
