Job description

MDS Assessor

The primary role of the RN MDS Assessment Coordinator is to assist in gathering of clinical documentation and completion of the non-comprehensive MDS assessments and ensures that essential information gathered during the assessment period, is integrated in the care planning process.

MDS Assessor Duties and Responsibilities

Administrative Functions

  • Complete and oversee MDS assessments as directed by Director of MDS
  • Ensure that the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process
  • Educate the interdisciplinary team about accuracy of clinical documentation and its importance in all aspects of care delivery
  • Coordinate the timely completion of the MDS assessments with the interdisciplinary team within the guidelines and specified time frames as per RAI Manual
  • Assist in the review of the clinical record accuracy of MDS coding and completeness of documentation.
  • Coordinate Case Management process
  • Act as liaison to the interdisciplinary team in the discussion of essential findings of a resident's condition obtained while completing MDS assessments
  • Assist in transmission of MDS assessments, and obtains validation records and reviews them for needed corrections
  • Other duties as assigned or as deem necessary and appropriate, as indicated by Director of MDS

Education

  • Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program

Experience

  • Experience in long-term care and one-year job-related experience in the MDS process
  • Must have familiarity with various computer software
  • Must be efficient, organized, detailed โ€“oriented and flexible
  • Must have excellent time management and consistently meet task deadlines
