Job description

Hazel Findlay is seeking an MDS Coordinator to coordinate and oversee functions of the MDS and resident care assessment process. We're looking for a compassionate, creative, and resourceful professional to drive our mission. Full-time position available.

Some responsibilities include:

  • Interview residents and others for information related to the MDS.
  • Review medical records to obtain information related to the MDS.
  • Review quality indicator reports for resident concerns.
  • Schedule MDS assessments according to state and federal guidelines.
