Department: Admin
Employment Type: Full Time
Location: St. Joseph, MI
Reporting To: Kara Gargano
Description
Schedule: Exempt (salary), min 40 hours per week | Monday - Friday | 8am - 4:30pm
Department: PACE of Southwest Michigan | St. Joseph, MI
This position requires a BSW, MSW, RN, PT or OT license.
What You'll Do:

Help connect older adults to the care they deserve. Join the PACE of Southwest Michigan team as you support the intake process for prospective participants, coordinating initial screenings and assessments under the guidance of the Enrollment and Marketing Supervisor. Your work will play a vital role in creating a positive first impression and helping individuals access comprehensive, life-enhancing care.
Key Responsibilities
  • Support the enrollment team in achieving program growth.
  • Responsible for all aspects of the intake process, including but not limited to; responding to inquiry calls, providing information and referrals, phone screening and in-home assessments.
  • Meets with prospective enrollees in their home.
  • Completes the Level of Care/ln Home Care Assessment on potential participants.
  • Effectively communicates with the team to coordinate referral related activities prior to enrollment.
  • Transmitting information to the State of Michigan.
  • Participates in the Interdisciplinary Team meetings.
  • Responsible for participant information input into various databases.
  • Coordinates timely follow-up with referral sources, families and participants through phone contact, letters and all other appropriate modes of communication.
  • Responsible for maintaining relationships with key referral sources.
  • Responsible for maintaining effective working relationships with agencies as related to the intake process.
  • Assists in community outreach activities.
  • Responsible for completion of in-home initial assessment.
  • Develops input to team on care plan development based on intake and Hierarchical Condition Category assessment.
  • Utilize assessment skills to determine eligibility of the participant for the PACE program.

Skills, Knowledge and Expertise
  • Completed bachelors' degree in a relevant healthcare field (such as nursing, occupational or physical therapy, social worker or other relevant clinical fields).
  • Active and unencumbered license/certification to practice in the State of Michigan as appropriate.
  • Two (2) years' experience in either home or community health.
  • Valid Michigan driver's license and reliable transportation.
