Hospice of Orange & Sullivan Counties is a community-based, nonprofit organization dedicated to providing compassionate, high-quality hospice and palliative care to patients and families throughout Orange and Sullivan Counties. Guided by our mission, we are committed to ensuring every individual receives dignity, comfort, and support during life's most challenging moments.

We are seeking a compassionate and detail-oriented RN Intake Coordinator to play a critical role in connecting patients and families with hospice services. As the primary clinical reviewer and coordinator for hospice referrals, you will help ensure patients receive timely access to the care and support they need while serving as a trusted resource to healthcare providers, referral sources, and interdisciplinary team members.

What We Are Looking For

Job Title: RN Intake Coordinator

Location: Newburgh, NY

Salary Range: $45.00 per hour

Schedule: Monday - Friday, 8:30 am - 4:30 pm

Position Summary

The RN Intake Coordinator serves as the primary clinical reviewer and coordinator for hospice referrals. This role evaluates hospice eligibility, facilitates timely admissions, supports referral source communication, coordinates with providers and interdisciplinary teams, and ensures a seamless transition from referral through admission.

The Clinical Intake Nurse functions as both a clinical expert and operational coordinator, balancing regulatory compliance, patient appropriateness, responsiveness, and exceptional customer service while supporting organizational growth and timely access to hospice care.

Key Responsibilities
  • Review and evaluate hospice referrals to determine eligibility and appropriateness for services based on hospice regulations and clinical criteria.
  • Coordinate the referral and admission process by collaborating with providers, referral sources, schedulers, and clinical field staff.
  • Serve as a clinical resource to hospitals, physicians, skilled nursing facilities, and community partners regarding hospice services and eligibility requirements.
  • Facilitate timely communication with patients, families, referral sources, and interdisciplinary team members throughout the intake process.
  • Ensure accurate and complete referral documentation while maintaining compliance with CMS, payer, and organizational requirements.
  • Prioritize and manage multiple referrals simultaneously in a fast-paced environment while maintaining a high level of customer service.
  • Support admission timeliness, operational efficiency, and agency growth through effective referral management and coordination.
  • Collaborate with clinical leadership and interdisciplinary teams to ensure safe and seamless transitions into hospice care.
