Job Title: Licensed Practical Nurse (LPN) - Cardiac Program - Immediate Hire
Location: Orange & Osceola Counties, FL

Matrix Home Care is a leading provider of home health services, committed to delivering high-quality, patient-centered care in the home setting. We are expanding our Cardiac Care Program and are seeking skilled and compassionate LPNs to join our growing team.
Position Summary:
The Licensed Practical Nurse (LPN) provides skilled nursing care under the supervision of a Registered Nurse (RN) to patients with cardiac and respiratory conditions in the home setting. This role is ideal for clinicians with experience in chronic cardiac care or those looking to grow in a specialized program.
Key Responsibilities:
  • Provide skilled nursing care in accordance with the established plan of care under RN supervision
  • Monitor and report changes in patient condition, including cardiac and respiratory status
  • Administer medications, including cardiac medications, as ordered
  • Perform treatments such as wound care, vital sign monitoring, and oxygen therapy support
  • Reinforce patient and caregiver education on disease processes and care plans
  • Recognize and report signs of cardiac or respiratory distress promptly
  • Maintain accurate and timely clinical documentation
  • Follow infection control and patient safety protocols

Requirements
Qualifications:
  • Active Licensed Practical Nurse (LPN) license in the state of Florida
  • Minimum 1 year of clinical experience (home health or cardiac experience preferred; willing to train strong candidates)
  • Experience with CHF, COPD, post-cardiac care (including CABG) strongly preferred
  • IV Certification preferred or willingness to obtain upon hire
  • Current BLS certification
  • Strong clinical, observation, and communication skills
  • Ability to follow care plans and work under RN supervision

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
