  • Develop and manage a tentative schedule of appointments, home visits, and progress observations, documenting significant advancements to monitor overall therapeutic outcomes.
  • Coordinate and conduct in-home visits that adhere to specific therapeutic protocols, focusing on individual needs through observations, chart reviews, and data analysis.
  • Implement and monitor program plans, ensuring that each therapeutic approach is executed effectively to support patient progress and well-being.
  • Participate actively in Interdisciplinary Team Meetings and contribute to collaborative discussions that inform case management and patient care strategies.
  • Prepare and file monthly and quarterly progress notes that capture patients’ therapeutic journeys and maintain comprehensive documentation.
