Key Responsibilities
  • Conduct comprehensive assessments to evaluate patients' mental health status and establish the appropriate course of treatment.
  • Maintain open and effective communication with team members, supervisors, families, and other collateral figures concerning clients' progress and needs.
  • Develop and provide a structured schedule of appointments, home visits, and therapeutic observations to monitor client advancement actively.
  • Coordinate and conduct home visits for in-depth observations, chart reviews, and staff interviews, as necessary.
  • Implement and ensure the fidelity of the treatment plans while actively participating in interdisciplinary team meetings to align care strategies.
  • Document patient progress meticulously through monthly and quarterly notes, ensuring transparent communication with all stakeholders involved in the care process.
  • Organize and lead training sessions to ensure all staff members are adequately informed about the program plan and documentation practices.
  • Provide continuous support through follow-up interviews and additional home visits based on patients' evolving needs.
  • Serve as a liaison to the community and collaborate with external organizations to further develop and refine therapeutic programs.
  • Develop procedures for training materials that enhance program effectiveness for clients.
Patient Population / Client Focus

As a Licensed Professional Counselor at Motion Matrix Counseling, you will work with a varied client population, including both adults and youth. Qualifications
