Matrix Home Care is a leading provider of home health services, committed to delivering high-quality, patient-centered care in the home setting. We are expanding our Cardiac Care Program and are seeking experienced and compassionate RNs to join our growing team.
Position Summary:
The Registered Nurse (RN) provides skilled nursing care, clinical oversight, and case management to patients with cardiac and respiratory conditions in the home setting. This role is responsible for developing and managing the plan of care, ensuring high-quality outcomes, and supporting patients with complex cardiac needs.
Key Responsibilities:
  • Perform comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing visits, including Start of Care (SOC), recertifications, and routine visits
  • Manage and coordinate care for patients with cardiac and respiratory conditions
  • Monitor patient status and respond to changes in condition, including signs of cardiac distress
  • Administer medications and treatments, including cardiac-related therapies
  • Complete and maintain accurate OASIS documentation and clinical records
  • Educate patients and caregivers on disease management, medications, and care plans
  • Collaborate with physicians, interdisciplinary team members, and caregivers
  • Ensure compliance with Medicare guidelines, agency policies, and regulatory standards

Requirements
Qualifications:
  • Active Registered Nurse (RN) license in the state of Florida
  • Minimum 1-2 years of clinical experience (home health or cardiac experience preferred)
  • Strong experience with Medicare patients and OASIS documentation preferred
  • Experience with CHF, COPD, post-cardiac care (including CABG) strongly preferred
  • Current BLS certification
  • Strong clinical judgment, assessment, and communication skills
  • Ability to work independently and manage a caseload

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
