Position Summary
Matrix Home Care is seeking experienced Registered Nurses (RNs) for weekend home health coverage in Pasco and Pinellas Counties. This position is ideal for nurses who are dependable, clinically skilled, and comfortable providing care in the home setting. The RN will provide skilled nursing visits, complete patient assessments, follow physician orders, document care accurately, and communicate with the clinical team regarding patient needs and changes in condition.

Schedule
Weekend coverage
Saturday and Sunday availability preferred
Per diem / PRN opportunities available
Responsibilities
Provide skilled nursing visits in the home setting.
Complete patient assessments and follow the established plan of care.
Provide wound care, medication management, disease education, and other skilled nursing services as ordered.
Monitor patient status and report changes to the Director of Nursing or clinical team.
Complete accurate and timely documentation.
Communicate professionally with patients, families, physicians, and the care team.
Maintain compliance with company policies, state regulations, and home health standards.
Requirements
Active Florida Registered Nurse (RN) license in good standing.
Minimum of 1 year of nursing experience preferred.
Home health experience preferred.
Experience with skilled visits, assessments, wound care, medication management, and patient education preferred.
Must be reliable, professional, and comfortable working independently in the field.
Strong communication and documentation skills required.
Must be able to pass all required background screening and onboarding requirements.
