Job Summary:
We are seeking a highly motivated and organized PAS Care Manager to join our team. In this role, you will play a crucial role in coordinating patient care, collaborating with healthcare professionals, and ensuring optimal patient outcomes. This position requires strong communication, organizational, and problem-solving skills, as well as a passion for patient-centered care.
Responsibilities:
  • Patient Care Coordination:
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive patient care plans.
  • Monitor patient progress, identify potential problems, and adjust care plans as needed.
  • Provide education and support to patients and their families regarding their health conditions and care plans.

  • Communication and Collaboration:
  • Maintain clear and effective communication with patients, families, and healthcare team members.
  • Facilitate communication and collaboration among different departments and disciplines.
  • Participate in team meetings and rounds to ensure coordinated care

  • Documentation and Record Keeping:
  • Maintain accurate and up-to-date patient records and documentation.
  • Ensure compliance with relevant regulations and standards.

  • Resource Management:
  • Identify and access appropriate resources for patients and their families.
  • Advocate for patient needs and ensure access to necessary services.

  • Other Duties as Assigned:
  • Perform other duties as assigned by the supervisor.

Qualifications:
At least two years of experience in care management, healthcare, or home care settings is preferred.
Licensed Vocational Nurse
Social Worker.
Skills:
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a team.
  • Proficiency in electronic health record (EHR) systems.
  • Knowledge of relevant healthcare regulations and standards.
  • Empathy, compassion, and a patient-centered approach.

Compensation: $20.00 - $25.00 per hour
