We are a highly rated, licensed, and certified home health care agency with CHAP accreditation. Our mission is to provide highly skilled and personalized care to patients through our dedicated team of professionals.
Summary
As an Intake Coordinator at Living Hope Home Healthcare, you will play a crucial role in ensuring that our patients receive the highest quality of care. You will be responsible for managing patient intake processes, verifying insurance information, and coordinating with healthcare providers to facilitate seamless patient transitions into our care program.

We are looking for weekends and holidays (Friday, Saturday, and Sunday), 9-6.


Responsibilities
Manage patient intake processes efficiently and accurately.
Verify insurance information and ensure compliance with HIPAA regulations.
Coordinate with healthcare providers to facilitate smooth transitions for patients.
Review documentation for completeness and accuracy.
Provide exceptional patient service throughout the intake process.
Maintain accurate records within EMR systems.
Assist in developing care plans based on patient needs.
Communicate effectively with patients and their families regarding intake procedures.]

Qualifications
High school diploma or equivalent required; an associate's degree preferred.
Experience in medical administrative support or a similar role is highly desirable.
Proficient in EMR/EHR systems; familiarity with Epic or Meditech is a plus.
Strong understanding of medical terminology and documentation review processes.
Knowledge of HIPAA regulations related to patient information privacy.
