Position Summary
Higher Ground IHS is seeking a knowledgeable, organized, and compassionate Agency Broker to support individuals enrolled in New York State OPWDD Self-Direction services. The Agency Broker will assist individuals and families with developing, implementing, and maintaining Self-Direction budgets and services while ensuring compliance with OPWDD regulations and person-centered planning principles.
This role requires strong communication skills, attention to detail, advocacy, and the ability to work collaboratively with families, care managers, Fiscal Intermediaries, and community providers.
Key Responsibilities
  • ย Assist individuals and families in understanding OPWDD Self-Direction services and optionsย 
  • ย Develop and maintain person-centered Self-Direction budgetsย 
  • ย Support completion and submission of Self-Direction documentationย 
  • ย Assist with staffing plans, community supports, and individualized goalsย 
  • ย Coordinate with Care Managers, Fiscal Intermediaries, brokers, and providersย 
  • ย Monitor budget utilization and service implementationย 
  • ย Ensure ongoing compliance with OPWDD guidelines and timelinesย 
  • ย Maintain accurate and timely documentation and case notesย 
  • ย Support participants with community integration and resource navigationย 
  • ย Attend Life Plan meetings and interdisciplinary team meetings as neededย 
  • ย Educate families regarding Self-Direction rules, flexibility, and responsibilitiesย 
Qualifications
Preferred:
  • ย Experience working with OPWDD Self-Direction servicesย 
  • ย Knowledge of OPWDD regulations and Medicaid Waiver servicesย 
  • ย Experience with person-centered planningย 
  • ย Strong organizational and communication skillsย 
  • ย Ability to manage multiple cases and deadlinesย 
  • ย Proficiency in Microsoft Office and electronic documentation systemsย 
Minimum Requirements:
  • ย High School Diploma or GED requiredย 
  • ย Associateโ€™s or Bachelorโ€™s degree in Human Services, Social Work, Psychology, or related field preferredย 
  • ย Valid NYS Driverโ€™s License and reliable transportationย 
  • ย Ability to travel locally within the Capital Regionย 
  • ย Background check and required clearancesย
