Job description

Position Summary: Responsible for coordination and implementation of comprehensive case management services for youth in the Maternity Home and compliance with DFPS minimum standards for licensing and contract items. Responsible for assisting and providing guidance and direction to case managers in residential programs.

Maternity Home Case Manager
โ€ข Proactive, creative, and flexible in-service delivery of case management services to residents in the Wesley Inn Program.
โ€ข Responsible for gathering information for potential intake for approval with a LCCA.
โ€ข Coordinate placement of each client with CPS Caseworker, Resident Advisor, and Parent or Guardian.
โ€ข Responsible for detailed completion of initial intake forms and admission documentation. Ensure new and current residents' information is entered into Extended Reach within 1 business day of admission including level of care.
โ€ข Maintain accurate and complete documentation for assigned clients, including monthly case notes for Moderate clients and bi-monthly case notes for Specialized clients.
โ€ข Maintain weekly individual case management sessions with each client.
โ€ข Complete initial and subsequent service plans in a timely manner.
o invites sent to all the professional team members at least 14 days prior to meeting.
o Initial service plan completed within 30 days of placement.
o Subsequent service plans completed within 90 or 180 days after previous service plan depending on county of placement and level of care.
o Completed service plans sent to CPS and Director of Residential Programs within 10 days of service plan meeting.
โ€ข Scheduled, oversee, and monitor resident(s) medical, dental, therapy visits, psychological and psychiatric appointments. Assist with transporting residents to activities and/or medical appointments.
โ€ข Coordinate referrals with community-based providers for additional services based on resident needs and eligibility (this includes expanding the existing referral network as necessary).
โ€ข Serve as liaison to outside agencies and managing conservators (primarily CPS and CASA).
โ€ข Responsible for preparing and distributing the daily program census and updating the CPS Provider Vacancy located on the DFPS site.
โ€ข Responsible for filling vacancies for residents when applicable and securing appropriate
โ€ข approvals and documentation from RCCL Licensing Representative
โ€ข Review all discharge plans with CPS case worker prior to resident discharge.
โ€ข Cooperate in licensing investigations in a positive and proactive manner.
โ€ข Review both Serious and Minor Incident reports for thoroughness and compliance with minimum standards. Ensure Serious Incidents are reported within 24 hours to Licensing and Director of Residential Programs
โ€ข Responsible for securing final approval and signature on all clients' plans of services by the Director of Residential Programs.
โ€ข Participate in clinical treatment team meetings with program staff, directors, and therapists.
โ€ข Provide appropriate crisis intervention when necessary, adhering to documented standards.
โ€ข Provide all services related to case management for assigned residents while in aftercare and complete follow-up information at three- and six-month intervals.
โ€ข Plan for psychological assessments, medical and dental evaluation, drug and alcohol assessments, school enrollment, DHS, and Medicaid appointments for clients on as needed basis.
โ€ข Requires on-call responsibilities to ensure an appropriate level of service for our residents.
โ€ข This position requires, in special situations, for the case manager to be counted in "Ratio" as a Staff member.
โ€ข Presents information and coordinates after care planning for youth staffing.
โ€ข Other duties as assigned by the Director of Residential Programs.

Education/Experience:
โ€ข Educational qualifications (at least one)
a) Bachelor's degree in social work or related field
b) 1-2 years in case management

โ€ข Experience working in an evidence based, trauma focused therapeutic environment is preferred.
โ€ข Must be at least 21 years of age.
โ€ข Knowledge of child development and behavior management
Working knowledge of DFPS Minimum Standards
