RN Maternity Case Manager

Schedule: Monday to Friday 8am-5pm

As an RN Maternity Case Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits and costly service gaps; and a less stressed, more effective health care system for us all. Want more flexibility, want more autonomy? This is a work from home position with working with the Healthy First Steps Program. Employee must reside in the state of Florida within the advertised region.

If you reside in the state of Florida, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Making outbound calls to assess members' current health status (potential for field visits as well)
  • Take incoming calls from members
  • Identifying gaps or barriers in treatment plans
  • Providing patient education to assist with self-management
  • Interacting with Medical Directors on challenging cases
  • Coordinating care for members
  • Making referrals to outside sources
  • Coordinating services as needed (home health, DME, etc.)
  • Educating members on disease processes
  • Encouraging members to make healthy lifestyle changes
  • Documenting and tracking findings
  • Making 'welcome home' calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:

  • Current and unrestricted Registered Nurse license in state of Florida
  • 3+ years of experience in a hospital setting, acute care, direct care experience within the OB and/or Maternity field
  • 1+ years of experience working with MS Word, Teams, Excel and Outlook
  • Reside in the advertised region.

Preferred Qualifications:

  • Bachelor's Degree
  • Case management experience
  • Background in managing populations with complex medical or behavioral needs
  • Experience with electronic charting
  • Bilingual - Spanish
  • Experience with arranging community resources
  • Field-based work experience
  • Knowledge of Florida Community Maternal Child Resources this ties into the need for recent maternity or OB experience

Soft Skills:

  • Demonstrated solid organizational skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
