Overview

Job Title: Medical Forms Designer
Location: Coppell, TX-on site
Job Type: Hourly Consultant
Hours: 8 am to 5 pm
Pay: $22.80/hr

Essential Functions:

  • Serve as the primary resource and advisor for medical record forms document design and revision initiatives.
  • Convert appropriate medical record forms to digital platforms and identify opportunities for digital document development and optimization.
  • Participate as an active member of the client’s Facility Form Committee to review, revise, and develop medical forms as needed.
  • Provide expertise and support for document production requirements across lithographic, web printing, and digital print solutions.
  • Manage customer relationships by advising document owners on cost-effective document production strategies and solutions.
  • Promote production and design print program services throughout the organization.
  • Upload forms into web ordering systems and coordinate print projects with customer service representatives and vendors.
  • Interface with preferred internal and external suppliers to ensure successful project execution and delivery.
  • Manage print and document workflows including forms committee coordination, proof exchanges, approvals, and delivery tracking.
  • Provide forms creation, revision, and print-on-demand (POD) design services.
  • Act as a creative consultant for forms layout, construction, and graphic design best practices.
  • Ensure all forms and documents maintain a high level of accuracy and compliance, recognizing the potential impact on patient care.

Skills:

  • Demonstrated experience with forms design software including Forms Fast, Adobe Creative Suite, and Adobe Acrobat Pro.
  • Strong interpersonal and communication skills with the ability to effectively present document strategies to senior leadership.
  • Ability to collaborate across teams and share benchmark solutions and best practices.
  • Excellent organizational, analytical, and problem-solving skills.
  • Strong attention to detail and accuracy in document creation and management.
  • Ability to manage multiple priorities and deadlines in a fast-paced environment.
  • Proficiency with PCs and Microsoft Office applications.
  • Strong interviewing and requirements-gathering skills to identify customer needs.
  • Ability to function effectively at all organizational levels.

Experience:

  • Minimum of three to five (3–5) years of forms design experience, preferably within the healthcare industry.
  • Minimum of three (3) years of experience in a customer relationship or client-facing role.
  • Experience with document production techniques including commercial, transactional, and digital print environments.
  • Proven ability to work independently and autonomously.
  • Strong forms and graphic design expertise required.
  • Customer service and vendor management experience preferred.
  • Print procurement experience with commercial vendors is helpful.
  • Excellent time management and project coordination skills required.

Education:

  • Minimum: High School Diploma or equivalent.
  • Preferred: Associate degree.

Excellent verbal and written English communication skills and the ability to interact professionally with a diverse group are required.
