Volunteers of America Upstate New York's continuum of residential programs offer homeless & low-income individuals and families safe housing and support services. We provide long-term solutions to the problems of homelessness, addiction, incarceration, and poverty by helping the people we serve overcome obstacles and develop healthier, more self-sufficient lives.

Case Manager - CBP

SUMMARY:

The case manager provides support services to individuals and families experiencing homelessness to secure and maintain stable housing and household income while enrolled in the VOA Permanent Supportive Housing Program.

ESSENTIAL JOB FUNCTIONS:

  • Provide all aspects of Case Management for a specified case load, in accordance with VOA policies, HUD and community standards.
  • Responsible for completing all HUD requirements, including but not limited to: Housing Quality Inspections, Rental Calculations, Rent Reasonable Certifications, Annual Assessments and Recertification's, Referrals and Service Planning.
  • Responsible for regular client meetings and engagement, including phone calls and home visits. Contact is at least once per month or more often as needed.
  • Responsible for life cycle of a client, from referrals, intake, housing search, move in, service planning, regular contact, annual recertification and graduations.
  • Create and maintain good working relationships with landlords
  • Provide ongoing and relevant pro social supports and life skills training
  • Use evidence based programming to help clients address challenges to housing stability.
  • Provide transportation to clients as needed.
  • Establish good working relationships with community based agencies and maintain a comprehensive referral network for clients.
  • Maintain accurate and timely records for all participants in accordance with VOA policies, relevant regulatory and funding sources and community standards.
  • Prepare reports as requested
  • Other duties as assigned to support the needs of the clients and program.

EDUCATION & QUALIFICATION REQUIREMENTS:

  • Associates Degree in Human Services or related field with relevant work or life experience preferred.
  • Clean NYS Driver's License
  • Strong organizational skills
  • Strong working knowledge of Microsoft Office software
  • Flexibility required to respond to emergency client needs.
