In our Youth and Family Services, we help parents and young people who are striving to become healthy and restore their relationships. We work in partnership with human services, child welfare systems, juvenile courts, and community-based providers so that together, we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.

We are currently looking for full-time Case Manager
Salary Range: $44,676 $47,650 (based on experience and education)

Summary This position is responsible for making all reasonable efforts to maintain medical records and a health care delivery system for children, who are 6 years of age or younger, in the legal care and custody of DCFS and who are placed in substitute care in the areas served by the agency.

Essential Duties and Responsibilities

  • Provide interim medical case management to all DCFS Wards in the Lead Agencys service area through the first 45 days of custody.

  • Contact substitute care giver quarterly to get updates on doctor appointments, specialty doctor visits, or emergency room visits.

  • Request all medical Records two months before the Annual Case Review.

  • Insure all children in DCFS care are up to date on dentist, vision, and hearing appointments.

  • Insure all children in DCFS care maintain a primary care physician.

  • Assist substitute guardian with obtaining services or resources such as WIC.

  • Complete an assessment with substitute care giver twice a year to make sure the child (ren) in DCFS care are emotionally and physically stabled.

  • Provide a Health Passport to the care giver when needed.

  • Complete an Annual Case Review report for every child in DCFS custody

Ideal Candidate Profile:

  • Bachelors degree in social work or a related field with two years experience in community health or maternal and child health.

  • Good communication and organizational skills.

  • Specific knowledge necessary for working with special population

  • Possess clinical skills to effectively manage clients

  • Knowledge of the Health Insurance Portability and Accountability Act or HIPPA

  • Good computers skills which include proficiency in Microsoft Office applications and the Internet.

  • Ability to work with a diverse population

  • Ability to work flexible hours and travel as needed (evening/weekend hours)

  • Must be available to attend meetings and meet the needs of the clients throughout the city, counties or wherever needed (a valid drivers license, current auto insurance, and reliable automotive transportation is required).
