General Description of Duties and Responsibilities:ย 

The Day Program Coordinatorย is responsible forย ensuring that theย appropriate levelย and type of day habilitation, life skills, and community integrationย supportsย are always provided for all individuals in the program. The Day Program Coordinatorย is responsible forย ensuring the ability of Direct Support Professional (DSP) staff to provide a safe, supportive, and therapeutic environment, fostering growth and independence for all individuals supported.ย 

The Day Program Coordinatorโ€™sย main roleย is to provide modeling, support, training, and oversight for all staff and individuals on caseload to ensureย appropriate supportย focusing on the unique needs of everyone supported. The Day Program Coordinator will be expected to serve as DSP staff when needed and is expected to be familiar with the unique needs of everyone supported, ensuring that all staff are effectively meeting those needs, and notifyingย Director/COO/CEO/Owner of any concerns regarding abilities in a timely manner.ย 

Duties:ย 

  1. Ensure that all staff are trained and comfortable in their roles (direct care and documentation), and able to always ensure the health and safety of everyone. Problem solve and provide support when needed for staff and individuals served, training staff on individualโ€™s ISP goals/objectives and ensuring follow-through working on said goals/objectives for everyone.ย 

  1. Assistย with coordinatingย scheduling ofย andย assistย with the gathering ofย information/documentation for annual planning meetingsย andย implementย Individual Support Plansย in a timely manner.ย 

  1. Ensureย completion of monthly/daily data collection, ensure that staffย completesย quality daily documentation,ย completeย accurateย monthly reviews of services andย submitย within expected time.ย 

  1. Oversee tracking and ordering of medications/medical supplies as needed and ensuring individualsโ€™ needs are met, review MARs on regular basis and ensure accurateness and completeness and ensure proper procedure for medications is followed by all staff and address any issues/additional training needed/medication errors.ย 

  1. Spend time with individuals one on one as needed.ย 

  1. Ensure completion of the daily census record.ย 

  1. Ensureย individualsโ€™ books are kept up to date and always meet DMH requirements andย Palmerโ€™sย Home Careโ€™sย standards.ย 

  1. Completeย new staffย training. Complete new employee checklists, ensuring all needed documentation is onย file,ย and all needed training is complete, and thatย new staffย are well prepared to meet the needs of the individuals they will support.ย 

  1. Ensure that calendars of activities, outings, etc. are being made collectively by staff and individuals, and coordinate/oversee fair scheduling of transportation for outings and appointments.ย 

  1. Ensureย staff are familiar with criteria for DMH reportable events and that any DMH reportable events are reportedย immediatelyย toย Director,ย maintainย regular communication withย Directorย regardingย individuals, and report any problematic behaviors toย Directorย immediatelyย or within 24 hours. Ensure reports areย submittedย to CMRO within allottedย timeframes.ย 

  1. Ensure completion of monthly safety checklist and drills as required by DMH in each program.ย 

  1. Withย Directorย oversight, create and manage staff scheduling, manage time-off requests, be the primary contact for call-ins, shift trades, etc., and run overtime through approval of CEO before approving.ย 

  1. Work to foster an environment of respect and individual empowerment in the program, providing support, and when needed, corrective action to enable staff to provide excellent care on a day-to-day basis in the home.ย 

  1. Work to ensure the rights of the individuals supported are respected and advocatedย forย and that supports are provided in a way that empowers the individuals. Engage in ongoing dialogue with individuals concerning rights/responsibilities and encourage self-determination and self-advocacy in healthy and sociallyย appropriate ways.ย 

  1. Coordinateย doctorโ€™s orders for any medication changes to theย agencyย RN for individualsย in a timely mannerย to ensure the MARs forย each individualย supportedย isย correct by the first of each month to ensureย appropriate documentation.ย 

  1. Continually review employee timecards to ensure accuracy and complete bi-weekly payroll review and timecard approvals.ย 

  1. Keep calendars up to date daily to informย Palmer's Home Careย team of whereabouts/availability when needed.ย 

  1. Have a cell phone that is answerable during working hours, and if a call is missed, will return the call to staff, management, and providersย in a timely manner.ย 

  1. Check and answer emails each day, Monday through Friday, or on workdays, on a priority basis and inย a timelyย and effective manner.ย 

  1. Complete other duties/tasks as assigned byย Director/COO/CEO/Ownerย to ensure individuals supported are receiving the best care possible.ย 

  1. Provide on-site supervision of the program, participants, and staff for an average ofย 40 hoursย per week, more when necessary to the smooth operation of the program.ย 

  1. Four-year college degree from an accredited Institution or four years of relevant experience in a supervisor role in a direct care setting. Must have at leastย Highย School diploma or equivalent.ย 

  1. Ability to pass a family safety care registry screening.ย 

  1. Ability to show strong work ethic, accept supervision and constructive feedback, as well as provide constructive supervision and feedback.ย 

  1. Reliability, punctuality, and flexibility in scheduling. The needs of the individuals being served must beย met at all times.ย 

  1. Interpersonal skills to engage inย appropriate interactionย with individuals served, team, and people in the community and/or other agencies and entities.ย 

  1. Ability to lift over 50 lbs.ย 

  1. Valid class E driver's license, and the ability to drive a van.ย 

  1. Ability to complete all agency required training and certifications withinย 90 daysย of hire (CPR/First Aid certification, Level One Med Aid Certification, Abuse/Neglect training, etc.) as required by DMH.ย
